Cutting Carbs and Exercising More but Still Not Losing Weight with PCOS? Here's Why

If you’re cutting carbs, exercising every day and still struggling with your weight with PCOS / PMOS, you’re not alone.

So many women are told:

“Just eat less.”
“Cut the carbs.”
“Exercise more.”

But when you’re dealing with PCOS, insulin resistance, hormonal imbalances, fatigue or chronic stress, simply pushing harder isn’t always the answer.

One of my clients, Sarah, came to me doing exactly that.

She was following a low-carb diet and exercising every day, but she was exhausted, constantly experiencing cravings and frustrated that her weight wasn’t changing despite feeling like she was doing everything “right.”

The problem wasn't that she needed more discipline.

We needed to understand what was actually happening in her body.

Why More Restriction Isn't Always Better

Carbohydrates aren't automatically the problem.

For women with PCOS, the quality, quantity and combination of carbohydrates can matter, but completely eliminating them isn't necessary or sustainable for everyone.

Instead, we can focus on:

✨ Pairing carbohydrates with protein, healthy fats and fibre
✨ Choosing nutrient-dense, minimally processed carbohydrate sources
✨ Supporting stable blood sugar throughout the day
✨ Understanding whether insulin resistance is contributing to symptoms

Exercise is important too, but more isn't necessarily better.

If you're already exhausted and exercising intensely every day, adding more may simply leave you feeling depleted.

A more balanced approach may include:

🏋️ Strength training
🚶 Regular walking and daily movement
🧘 Lower-intensity or restorative movement
😴 Adequate recovery and sleep

What About Insulin Resistance?

Insulin resistance is common in PCOS and can make weight management more challenging for some women.

That's why I don't believe in giving every woman the same “PCOS diet.”

Instead, I look at the bigger picture—including your symptoms, blood sugar regulation, metabolic health, hormones, nutrition, sleep, stress and lifestyle.

For Sarah, changing her approach meant moving away from extreme restriction and excessive exercise and towards a more balanced, personalised strategy.

The result wasn't another unsustainable diet.

It was an approach she could actually maintain.

If you're doing everything “right” but still feel stuck with PCOS weight management, it may be time to stop trying harder and start looking deeper.

Your body isn't failing you.

You may simply need a different approach.
